I request a waiver of $150.00US one time virtual domain setup fees and agree that my domain (domain name): ____________________________ will be hosted by Sulima Internet for not less than 12 months at the regular monthly rate of $24.95 per month. No other hosting option qualifies for setup fee waiver. If I decide to move my domain within the 12 months period, I will be liable to pay the full setup fee of $150.00 unless otherwise mutually agreed by both of us (Sulima Internet and myself). I further understand that monthly payment is due on the 1^st of each month.

With this options, I also understand that I must make a total initial payment equal to three monthly payments or the sum of 74.85 within 7 days of mounting the domain.

Late charges apply after the 10^th of the month. There is no separate monthly billing. You may request one.
